Transfers from Other Hospitals

Call the Transfer Center at (650) 723-7342 and your call will be appropriately directed. If you require a Lucile Packard Children's Hospital Stanford Attending Physician, the Transfer Center staff will facilitate the process for you. Any transport requirements for your patient can also be addressed by calling the number above.

Please have the following information available when calling in your admission:

• Patient’s name
• Lucile Packard Childrens' Hospital Stanford medical record number (if known)
• Date of birth
• Diagnosis
• Referring physician phone number, name of facility/hospital, and patient location
• Fax sheet available for faxing upon request

The Admitting Department will be notified of your patient's pending admission.

Parents/responsible parties will be asked to come to Admitting to sign required admission documents.
