Want to recognize a staff member?

Our doctors, nurses and administrative staff members are all committed to providing exceptional care. If you feel like a staff member went above and beyond, please help us recognize their dedication. You can use our feedback form or mention that staff member in the Patient Experience Survey that you receive from us. Either way, we will ensure the staff member and leader has the recognition shared with them.. You may also reach out to a social worker or chaplain, or contact Office of Patient Experience at (650) 498-4847.

The DAISY Award for extraordinary nurses recognizes the super-human work nurses do every day. You can nominate an extraordinary nurse with the following DAISY Award nomination form in English or use the DAISY Award nomination in Spanish.

Nominations received by the 15th of the month will be considered for the following month's DAISY Award.

Want to share your thoughts or experiences?

After your visit, you will likely receive a patient experience survey from a 3rd party service, Press Ganey, to evaluate your experience with us. This survey will take 5-15 minutes of your time and helps us evaluate all aspects of your care experience. Your unique experience and feedback matters to us. Every voice counts. It will be used to improve the care patients and families receive from us in the future. We keep your personal information and feedback confidential. It may come to you via mail within a few days and up to a week after discharge, or within 1 business day of discharge via text or e-mail.

By completing a Patient Experience Survey, we will be able to:

  • Continue to be proactive in improving our services for all patients and families.
  • Celebrate and Recognize staff members, and more accurately understand your experience at our hospital.
  • Evaluate improvement efforts continuously ongoing within the hospital.

Patient Navigation

Patient Navigation is here to facilitate resolution for any concern or issue patients and family members may have with their experience. We strive to provide compassionate care and professional presence, while utilizing problem solving skills and effective communication to achieve resolution to concerns.

If at any time you have comments, questions, or concerns about your child’s care, we encourage you to talk to your nurse, physician, or the manager of your hospital unit so that they can get you the information that you need. We also have an excellent team in Spiritual Care and Social Services, that you are welcome to reach out as well through your nursing team on your floor. 

If you do not reach the resolution you are looking for with the unit staff, or you would prefer additional support in resolving your issue, please contact our office through any of the contact methods below.
