1. To get started, we will need a written referral from your physician. Please have your healthcare provider fax the completed form, along with any necessary lab results to:

How to Prepare
If you are having a nuchal translucency (NT) ultrasound or chorionic villus sampling (CVS), please drink 16 ounces of water 45 minutes before your procedure.
What to Bring
Please bring the following items to your Perinatal Diagnostic Center appointment:
- Insurance card
- Photo ID
- List of any medications you are taking
After Your Appointment
The results of your patient's tests or a summary of a consult or genetic counseling meeting will be sent to you as soon as possible. Results of nuchal translucency and level 2 ultrasound will be reviewed with you by a perinatalogist at the end of the appointment.
