Schedule your first appointment

When calling to schedule your first appointment, be sure to have your health plan information available. If we can’t fit you in on the day of your choice, we may offer you an appointment with another doctor in the same office or refer you to one of our other Bay Area locations.

Before your first appointment, please ask your new office about forms you can complete in advance to save you time on the day of your visit.
