Stanford Medicine Children's Health — Pediatrics (Formerly Pediatric Associates) is dedicated to providing the best in pediatric care to families throughout Santa Clara County. All of our clinicians are certified by the American Board of Pediatrics and are members of the American Academy of Pediatrics. We are here to help guide you with your children’s health care needs and believe in working with you to ensure that your children maintain optimal health as they grow and develop.

CHADIS Login

CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office. After you complete the questionnaire, CHADIS results come directly to your doctor. The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.

Take the CA-85 South exit toward Santa Cruz/Cupertino. Continue onto CA-85 South. Take the Los Gatos Boulevard/Bascom Avenue exit. Turn right onto Los Gatos Boulevard. Take the first left onto Samaritan Drive. 2577 Samaritan Drive will be on your left.

From Highway 101 North
Keep right at the fork to continue on CA-85 North; follow signs for Cupertino/Mountain View. Take the Union Avenue exit. Turn right onto Union Avenue. Take the first right onto Samaritan Drive. 2577 Samaritan Drive will be on your right.

From Highway 280 South
Take the exit onto CA-85 South toward Gilroy. Take the Los Gatos Boulevard/Bascom Avenue exit. Turn right onto Los Gatos Boulevard. Take the first left onto Samaritan Drive. 2577 Samaritan Drive will be on your left.

From Highway 880 South
Continue onto CA-17 South. Take the exit toward Gilroy. Merge onto CA-85 South. Take the exit toward Union Avenue. Turn right onto Samaritan Drive. 2577 Samaritan Drive will be on your right.

From Highway 680 South
Continue onto I-280 North. Take the exit onto CA-17 South toward Santa Cruz. Take the exit toward Gilroy. Merge onto CA-85 South. Take the exit toward Union Avenue. Turn right onto Samaritan Drive. 2577 Samaritan Drive will be on your right.
