For Referring Physicians

Since many of our patients travel to us from around the nation and world, we rely on building collaborative relationships with referring physicians who can pick up where our care visits leave off. While patients and families may see us for a comprehensive evaluation and complex heart surgeries or heart transplants, we depend on you to provide consistent, daily care, viewing us as a resource.

What to know when referring a patient

• Complete a Patient Referral Form (PDF) and fax it to our team with insurance information and relevant clinical documents (notes, imaging study, and lab results, etc.) at (650) 721-2884.
• Once your referral is received, a member of our team will reach out to you to discuss your patient.
