CHADIS Login
CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office. After you complete the questionnaire, CHADIS results come directly to your doctor. The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.

From I-5 South
Head south on I-5 South. Take the exit toward Tracy. Merge onto I-205BUS/West 11th Street. Turn left onto Chrisman Road. Turn right onto West. Linne Road. Turn right onto South Tracy Boulevard. 4598 South Tracy Boulevard will be on your right.
From I-5 North
Head northwest on I-5. Continue onto I-580 West (signs for Tracy/San Francisco). Take the Corral Hollow Road exit. Turn right onto Corral Hollow Road. Turn right onto West Linne Road. Take the first left onto South Tracy Boulevard. 4598 South Tracy Boulevard will be on your right.
