Overview
Important news regarding our practice closure on August 31, 2026.
The doctors that work at this clinic will continue to care for their patients at other South Bay Stanford clinics.
To make sure your child is still able to continue seeing the same doctor, their care will transition to their doctor’s new office. The new office is still within Stanford Medicine Children’s Health, and you do not need to do anything to transition your child’s care. If you have any appointments scheduled for after August 31, 2026, the clinic will call you to tell you the location has changed.

CHADIS Login
CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office. After you complete the questionnaire, CHADIS results come directly to your doctor. The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.

Follow I-280 South and CA-85 South to Los Gatos Boulevard. Take the Los Gatos Boulevard/Bascom Avenue exit from CA-85 South. Take National Avenue to Los Gatos – Almaden Road in Los Gatos. Turn right onto Los Gatos Boulevard. Take the first left onto Samaritan Drive. Take the first right onto National Avenue. Turn left onto Los Gatos – Almaden Road. 15899 Los Gatos – Almaden Road will be on your left.

From CA-85 South

From US Highway 880 South
Follow I-880 South and CA 17-South to Lark Avenue in Los Gatos. Take the Lark Avenue exit from CA-17 South. Take Los Gatos Boulevard to Los Gatos – Almaden Road. Turn left onto Lark Avenue. From CA-17 North
Take the exit toward East Los Gatos. Merge onto Saratoga Los Gatos Road. Turn left onto Los Gatos Boulevard. Turn right onto Los Gatos – Almaden Road. 15899 Los Gatos – Almaden Road will be on your left.
