Request or inspect your medical records
We provide access to clear and complete information so you can make the best possible healthcare decisions. Requesting a copy of your medical records is easy, whether for your personal records, or to be sent to another medical facility.
Lucile Packard Children's Hospital is required, under the State of California's Confidentiality of Medical Information Act, to obtain an original and complete Authorization for Release of Health Information form before providing a copy of a patient's records or allowing inspection to anyone, including the patient or their representative. In certain cases a patient's physician, psychologist or social worker may also be required to approve a request.
Onsite at the hospital
Please visit the Welcome desk at either the Main or West Buildings and a team member will be happy to assist you with requesting your records.

Be sure to provide the exact dates of treatment for which you are requesting information. Indicate the type of records you need and be sure to initial requests for information regarding AIDS/HIV, Psychiatric Care, or Alcohol/Drug Abuse.
Required adolescent consent
All record requests for protected confidential and sensitive services for Adolescent patients, aged 12-17 require a signed consent from the adolescent. Records will be reviewed by the provider prior to their release. Providers have the right to deny release if deemed appropriate. If you would like to request this information please complete section B.1 with dates of service and have patient sign section I, on page 6.
While you may request the entire medical record, the summaries provided in the Discharge Summary, Operative Report, and History & Physical contain the most frequently requested information. If you are not sure what information you need, call us at (650) 497-8079.
Did you know you can request your records thru your MyChart account?
Log on and select Your Menu located on the left hand side of the screen. Then Under My Records select Request Medical Records.
Here you will find more information about submitting your online request and complete the form to submit to HIMS for processing.
If you do not have a MyChart account, visit mychart.stanfordchildrens.org and click on the "Sign Up Now" button.
Follow these easy steps to request your records.
1. Log on to your MyChart account.
2. Select “Your Menu” and then select “Request Medical Records”
3. Complete the online form and submit to our department for processing.
Requesting your records in Person?
Our office is CLOSED for walk in service. Kindly call us at (650) 497-8079 and we can provide additional information and options for release of information.
Please allow 5 to 15 business days for requests to be processed. Once processed we will mail the records to the address provided or if you would like to pick up the records, we will contact you once they are ready. A photo ID is required in order to pick up records. If you are not the requestor and would like someone else to pick up the records, you will need to provide a note stating ok to release records to another party along with your signature.
Please note that radiology images, telemetry tapes and photos are not part of your medical record (your official records contain the reports that interpret those images, tapes and photos). Your request will be forwarded to the Radiology Film Library for processing. (650- 497-8376, option 2)
Medical records do not contain billing statements. Visit Financial and Insurance Information for instructions on how to obtain copies of your bills.
Inspection of medical records must be requested in writing and is done by appointment upon approval of request. Packard Children’s has the right to charge a fee for inspection of records. Please contact our Health Information Management Systems (HIMS) office for more information: (650) 497-8079.
If an individual other than the requestor is picking up the records, then that individual will need:
- An original signed authorization letter from the requestor
- A photo ID
Fees
- There may be a retrieval charge for inspecting your records. There may also be a reproduction charge for creating a copy of your records (including electronic records).
- If the medical record is being mailed to you, then you will also be charged for mailing costs.
- If the reproduction charges are greater than $50, then pre-payment will be required before your records can be processed.
- There is no fee to send records directly to another medical facility or medical office.
Health Information Exchange
Stanford Medicine Children’s Health participate in secure electronic health information exchange. Our ability to safely share health information with other members of your care team benefits you in many ways.
You may request to be exempted from sharing your health information via our health information exchange program. Note that request for exemption will not prohibit Stanford Medicine Children’s Health from sharing information with each other for permitted purposes.

Protecting your health information, including reproductive health information
In June 2022, the U.S. Supreme Court’s decision to overturn Roe v Wade led to legal restrictions against some reproductive health services in many states across the U.S. Stanford Medicine operates in California, where laws protect and enable comprehensive reproductive services for our patients.
No matter where you live or receive care, you may wish to limit access to information about your personal health. This document outlines actions you can take to protect the privacy of your health information, including your reproductive health information.
