What is the Stanford Medicine Children’s Health MD Portal? 

MD Portal connects community physicians and their staff to Stanford Medicine Children’s Health Epic electronic health system to view information about their patients being cared for at a Stanford Medicine Children’s Health location.

Who can get an account?

Providers and support staff.

What can you do in the portal?

Providers and support staff can get access to the full patient chart, submit and track the status of referrals, view radiology procedures, securely communicate with the care team, and view patient updates in real-time.

Other ways to submit an electronic referral

(Care Everywhere Referrals Management–CERM this must be enabled by your organization):

Make a referral request through Epic by selecting the following:

  • Class: External
  • Place of service/location starts with: Stanford Children’s Health
  • John Muir Health (JMH)
  • Sutter Health
  • University of California San Francisco (UCSF)
  • Stanford Health Care (SHC)
  • North East Medical Services (NEMS)

While our preferred method for receiving referrals electronically is via the above MD Portal or Care Everywhere Referrals Management (CERM), we have also launched digital versions of a few forms, streamlining your referral process.

By submitting the form electronically directly from your computer, or from a mobile device, we have eliminated the need to fax. You have the ability to attach relevant clinical documents, authorizations, and insurance cards, with the option to upload up to 30 files.
