Referring a patient

We welcome the chance to partner with you to care for your patient who may benefit from reproductive genetic testing and care.

To refer a patient, please complete our Perinatal Diagnostic Center Ultrasound & Genetic Counseling Order Form and submit it online to the PDC (Perinatal Diagnostic Center) with insurance information and relevant test results.

Once the referral is received, please have your patient contact our office to make an appointment at one of our Perinatal Diagnostic Center locations.
