Overview

Stanford Medicine Children's Health — Pediatrics (Formerly Peninsula Pediatric Medical Group) in Burlingame is dedicated to providing the highest-quality care to our patients. We specialize in the care of children from birth to the late teen years. Our team of pediatricians and specially trained medical office staff have earned a reputation for excellence, serving generations of families in the Bay Area. All of our pediatricians are certified by the American Board of Pediatrics.

If your child is a new patient, you will be asked to complete the following form.

Well-child visit forms

At some of your child’s regular check-ups, you will be asked to fill out a questionnaire about your child’s health. You can complete these forms in advance by downloading the appropriate questionnaire, filling it out and bringing the completed form to your child’s appointment. Please note that not all recommended check-ups will have forms.

CHADIS Login

CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office. After you complete the questionnaire, CHADIS results come directly to your doctor. The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.

From Highway 280 North
