Referring a Patient for Stem Cell Transplantation

We work closely with referring providers to care for patients before, during, and after stem cell transplantation.

To refer a patient for a potential stem cell transplant, please email DL-SCTIntakeCoordinators@stanfordchildrens.org and they will send you an intake form.

To refer a patient for post-transplant care, please fax the referral to (650) 721-1086. If you have any questions, please call (650) 497-8953 and select option 4, then option 1.
